To positively impact the lives we touch.
About us
Adobe Population Health (formerly known as Adobe Care & Wellness) began as an idea for a healthcare solution dedicated to improving lives. Applying the human touch, Adobe develops real relationships forging a pathway forward improving both the human and health condition of each member. Adobe builds trusted relationships by meeting members where they feel safe and comfortable, in their own homes (or as we like to say, their adobe). Beyond identifying at-risk populations, Adobe proactively intervenes through a customized data-driven approach meeting compliance requirements by closing care gaps, addressing social determinants and supporting transitional care management. Adobe does more than check a box; the innovative Adobe Care Management Model delivers whole-person care that changes lives.

Adobe Population Health's Perinatal Program is changing lives, and we have the results to prove it. After just one year, we've helped our Medicaid clients in New Mexico meaningfully outperform statewide averages, significantly reducing premature births from 10.1% to 7.3%, low birth weight from 9.7% to 8.2%, and NICU admissions from 11.7% to 7.3%. Our infant mortality rate was half the state average (0.28% vs. 0.59%), and we achieved zero maternal deaths within six weeks postpartum compared to the state’s rate of 0.028%. This is what proactive, whole-person maternal care can accomplish. Maternal health outcomes reflect more than individual cases. They signal the overall health of communities and the effectiveness of healthcare systems. In her latest commentary, Jayme Ambrose, DNP, CEO of Adobe Population Health, highlights the urgency of addressing social determinants of health proactively to improve maternal and infant outcomes. Adobe's hybrid care model, powered by MASLOW risk assessment technology, enables early identification and strategic intervention in SDOH-related risks, resulting in significantly improved clinical metrics and reduced maternal health disparities.
